This is an exceptional opportunity for a Family Practice Physician to join a busy, well-respected practice, supported by a strong referral base and the full resources of a robust health system network.
•
Join a collaborative, multidisciplinary team with 3 Physicians and 2 APP's along with RN and support staff
•
Access to onsite lab and Xray services
•
Call structure with other sites: 1:17
•
Practice setting: Outpatient clinic; average of 15 patients per day, serving patients of all ages with flexibility to increase volume based on provider preference and community need
•
Employed position within a nonprofit health system
•
Deliver comprehensive patient care across outpatient and inpatient settings, including evaluation, diagnosis, treatment, care coordination, and consultation, with opportunities to manage referrals and hospital-based responsibilities as needed
•
Engage in quality improvement, cost-effective care initiatives, and academic activities as desired, while maintaining timely, accurate documentation and collaborating with a multidisciplinary team

Care for a loyal and growing patient base in Little Chute, WI with a population of 12,000 and surrounding population in the Fox Valley of 80,000

What Minimum Requirements You'll Need Licensure / Certification / Registration:
•
One or more of the following required:
•
Physician MD/DO credentialed from the Minnesota Board of Medical Practice obtained prior to hire date or job transfer date.
•
Physician MD/DO credentialed from the Wisconsin Medical Examining Board obtained prior to hire date or job transfer date.
•
Advanced Life Support preferred.
Education
•
Doctor of Medicine (MD) or Doctor of Osteopathy (DO) required.
